Has The Stress Of Covid-19 Caused Your Acne To Flare Up?

Integrated Dermatology Offers Acne Solutions During the Pandemic

Stress has affected us all during the pandemic. Coping with the crisis, being quarantined or in isolation, or home-schooling your kids are all factors that are causing high levels of stress for many. While a little bit of stress is okay, too much stress can wreak havoc on your body mentally and physically.

Stress can have an impact on your skin, exacerbating many dermatologic conditions, such as acne, eczema, hair loss, and psoriasis.

“’Don't stress too much’ is so much easier said than done, especially given today’s circumstances,” says Allison Senerchia, a Physician Assistant with Integrated Dermatology in Washington DC. “Instead, get your body moving for just a few minutes each day. Whether it's a walk around the block or a 5-minute yoga stretch, anything you can do to get some endorphins flowing will hopefully reduce some stress and lead you on a path of clearer skin.”

“Your skin will let you know when your body is overwhelmed with stress,” said Dr. Agnes Ju Chang, Medical Director of Integrated Dermatology. “It causes inflammation, triggering stress zits and making your current skin conditions worse.”

           Acne

is the most common skin condition in the US, affecting both teenagers and adults. Adult acne can be very frustrating and distressing. The cause of acne is multifactorial in nature and involves oil glands, bacteria, hormones, and lifestyle factors.

“Stress can trigger cortisol, which is our fight or flight hormone,” explains Dr. Sarika Snell. “This hormone can affect the skin by stimulating our oil glands to increase more oil and trigger inflammation in our skin that can lead to acne. As dermatologists, we can help you by creating an acne regimen that will help reduce the inflammation and oil production in the skin, which will then improve the acne for beautiful and clear skin.”

There are very effective treatments for acne that may be helpful to clear up your skin. These include topical treatments (applied to the skin), systemic treatments (which work throughout the body), and procedures. Lifestyle factors such as nutrition and sleep should also be considered.

Topical treatment consists of a sound skin care regimen and incorporating prescription creams. Systemic treatment consists of oral medications, such as antibiotics, spironolactone, and isotretinoin

           (Accutane)

. Procedures including chemical peels and lasers can also be helpful for hyperpigmentation and acne scars.

Teledermatology is a convenient and easy way to connect with your healthcare provider to discuss your skin concerns, symptoms and more. This service involves having a virtual video consultation with your health care provider in real-time.

While telemedicine is not appropriate for acute or emergency situations where immediate hands-on assessment and treatment are necessary, for routine straightforward conditions, it may be helpful.

Exposure to blue light throughout the day is inevitable in our modern society. We use our smart phones, laptops, and tablets more than ever before. This connectivity may increase our productivity, but new research shows that blue light may have negative impacts on our skin. Washington DC’s top dermatologists at Integrated Dermatology share tips for minimize thes negative effects of blue light on your skin.

Unplugging is beneficial for your mental health, but did you know it also has physical benefits? “There is not currently enough solid research to know the totality of the effects of blue light, but we know it does play a role in the aging process,” says Dr. Agnes Chang with Integrated Dermatology in Washington DC. “I am on board with anything we can do to slow down the process.”

It is almost impossible to avoid screen time these days, but it is a good idea to take a critical look at the amount of screen time you’re using. Some phones and devices will send you alerts if you are using your phone or tablet more than usual, giving you an opportunity to reduce usage. Dr. Chang has found it easier to do this by eliminating screen time at least 30 minutes before bed. Limiting exposure to your smartphone and tablets/laptops is so important for your natural circadian rhythm. Melatonin production is important for restful sleep, and having too much blue light exposure will decrease the melatonin level in your body. This can lead to an increase in cortisol levels at night (stress hormone) which can directly impact your skin. New research shows that restful sleep has a profound effect on general health. “You can always tell when someone is sleep deprived,” says Integrated Dermatology’s Medical Director Dr. Agnes Chang. “Their skin tends to look duller, sallow, and dehydrated.”

USE PRODUCTS THAT PROTECT YOUR SKIN

Some products will protect against UV light, but in addition they can be a shield for your skin against blue light. Antioxidants are crucial for protection against blue light and environmental exposure. “One of my favorite products to prevent against blue light damage is SkinMedica’s Lumivive System,” says Victoria Hatfield, PA-C with Integrated Dermatology. “It is specifically formulated antioxidant with ingredients to protect and repair your skin. I’ve seen noticeably brighter skin on myself and my patients since starting this product.” Another favorite is Skinceuticals C E Ferulic, which can act as a shield against environmental damage.

           DON’T FORGET ABOUT YOUR EYES

Our eyes absorb a lot of direct exposure to various types of light. “With the evolving technology around us, it is increasingly important to protect against blue light,” says Hatfield. “I personally recommend blue light blocker glasses when using a smart phone or looking at the computer. I suffered from headaches due to prolonged screen exposure and with the use of blue light blocker glasses, they have significantly decreased.”

Q: What led you to choose dermatology?

A: Out of all the specialties in medicine, dermatology was my absolute favorite. I think I loved the relationships that you build with your patients and the pure joy and tremendous satisfaction you get when a patient feels how good healthy skin looks. Plus, I love the artistry involved in cosmetic and surgical dermatology, it’s very exciting and fun for me as well. It was really an easy choice when I think about it!

Q: Is there a specific patient or a patient experience that has made an impact on you?

I think you always remember the first patient who you diagnosed with a skin cancer. It is the most important aspect of dermatology and our ultimate goal is always protecting patients from skin cancer when we are examining the skin. For me, it is a reminder of how much I appreciate patients who put their trust in me to manage their health. I have also loved every moment when a teenager gives me the biggest smile because their acne is clear, and they are so happy. Or, every time someone suffering from a full body skin rash finally gets relief. And of course, I must admit, the excitement from a cosmetic patient when their Botox finally kicks in never gets old either.

Q: What would you do if you were not a Dermatology Physician Assistant?

A: I love animals so much, I’d probably be a vet.

Winter is the time of year for dry skin. It is cold and windy outside, and the humidity level is low. We use heaters in our cars and inside our homes, which continue to dry out our skin. The Mayo Clinic describes dry skin as a lack of natural oils. Some of the tell-tale signs of dry skin are:

“Flakiness is my first clue to reach for my SkinMedica HA5 or Kiehl’s Intensive Repair Moisturizer,” says Integrated Dermatology’s Dr. Lauren McKelroy. Ignoring flaky skin can lead to itching or chapped looking skin.

Our skin functions as a barrier against germs and outside pollutants. So, what is the best way to ensure it as strong and healthy as possible? Integrated Dermatology’s Dr. Agnes Chang recommends creams that naturally draw moisture to the skin. “Look for ingredients such as glycerin and hyaluronic acid, which attract water when applied to the skin,” she advises.

Nicole Masson, a Physician Assistant with Integrated Dermatology, follows the, “Ooey Gooey” rule, stating: “Products that are thick and sticky are the best for dry skin. For dry hands I typically recommend using plain vaseline and layering gloves on top for at least 20 minutes a day.”

Protection is the best defense in this situation, but what if you already have cracked skin or are experiencing itching? When symptoms become painful or uncomfortable, it’s time to make an appointment with your dermatologist, as prescription medications might be called for.

Water is essential to healthy, normal bodily function and healthy habits include drinking the daily recommended amounts of water. Dr. Chang shares that she aims to drink half her body weight in ounces of water every day. For example, if you weigh 160 lbs., you should aim for 80 oz. of water a day for basic metabolic function. Healthy habits are the foundation of healthy skin, and water is an important building block to optimal skin health. “Your skin will give you clues that water intake is vital,” says Dr. Chang.

An Interview with Integrated Dermatology’s Medical Director

Q: What led you to choose dermatology?

A: When I went to medical school, I wanted to find a specialty that would allow me to use my clinical skills to make a significant impact in the lives of people. What I love about dermatology is that it is a “visual” specialty that allows me to look at the skin and diagnosis the condition. There is nothing as gratifying as when the patients come back for their follow up after being treated by me and there is a visible improvement with their skin. I’m able to help improve the quality of life of people in a real and tangible way. The skin is amazing in that it is also a window to how the rest of the body is doing. I also love the fact that you can help people look their best. A little bit of Botox, fillers, and a chemical peel can take you a long way. I am happy when people say “wow” when they see their reflection after their treatment! love what I do and hopefully it shows!

Q: Is there a specific patient or a patient experience that has made an impact on you?

A: When I was a fourth-year medical student, I rotated through the oncology ward. There was a woman there receiving chemotherapy for her metastatic melanoma. She told me her story. She had a mole on the bottom of her foot that she had ignored for a long time. When it started to change, at the urging of her family, she went to see a dermatologist. By the time the biopsy showed melanoma, it had already spread to other organs. Her story made a big impact on me. At the time of that clinical rotation, I hadn’t even considered dermatology. It’s amazing that after all those years, I get to help my patients with melanoma prevention. I think about that lady from time to time and the deep impression she made on me.

Q: What would you do if you were not a dermatologist?

A: I never believed in the saying that if you put your mind to it, you can be anything. I mean, I would love to be a world famous ballet dancer, but I don’t think my body would move that way. Having said that, I think it would be really cool to be a motivational speaker and life coach where I would get to share my story and uplift the lives of others.

Q: What’s your favorite procedure to perform?

A: I don’t know if you would categorize this as a procedure, but I love performing a full body examination. It screens for skin cancer and I have an opportunity to educate my patients about their skin health. I have a lot of skin cancer patients that see me pretty regularly and I get to hear about their life and stories, which I enjoy tremendously. I get to establish amazing relationships with patients, and that’s awesome.

Q: What’s one thing people often neglect in their skin care regimen?

A: I think that the skin is a window to what is going on with the rest of the body. As a board-certified dermatologist, I am trained to diagnose and treat skin conditions. However, I think it is important to not neglect your general health and how it affects your skin. Nutrition and lifestyle are important in the treatment of certain skin conditions. For example, dairy and sugar can cause inflammation and trigger acne. Sure, I prescribe medications when necessary, but I also believe that you have to be accountable and responsible for your nutrition, habits, and lifestyle. I like to talk about the three “S’s”—sugar, stress, and sleep—with my patients and how they affect the skin. I also emphasize the importance of the partnership between myself and the patient to get their skin to look its best.

BOTOX® Cosmetics and DYSPORT® are injections that prevent and decrease the appearance of fine lines and wrinkles. BOTOX® is preventative

. More people are open to discussing non-invasive cosmetic procedures, but many wonder when they should start injectables such as BOTOX®. “Baby Botox” or “Microtox” are treatments that use small amounts of BOTOX® to prevent lines from setting in.

Washington DC’s Integrated Dermatology Medical Director, Dr. Agnes Chang, started personally using Botox in her late 20’s. “I feel that preventive measures to decrease wrinkles are the best way to diminish the signs of aging,” she says. “Does it stop the aging process? No, but Botox helps the organic aging process be as graceful as possible.”

           BOTOX® is a medical treatment.

The most common areas treated with BOTOX® and DYSPORT® are the forehead lines, the area in between the eyebrows, and the lines around the eyes (crow’s feet). These areas have been approved for treatment by the FDA. These prescribed amounts have been proven through research to give optimal results.

During a cosmetic consultation with one of Integrated Dermatology’s leading cosmetic dermatology providers, we take into account the unique features of our patients, as well as the “look” they want to achieve, and we develop a customized plan. It is advised to take caution against trying to save money by cutting back on the amount of BOTOX® that is recommended. This will lead not only to unsatisfactory results, but also the effect of the BOTOX® will not last as long. In fact, recent studies with identical twins provide a visual guide into the effects of not treating according to the prescribed dose. Here is an example of twin patients that were treated by our Integrated Dermatology’s Medical Director, Dr. Chang.

There can be a misconception that BOTOX® or DYSPORT® look unnatural, but that speaks to the skill level of the person holding the syringe. Choose an injector with a degree and training in facial anatomy, as well as medical experience. It is important to choose an expert injector, such as one of the board-certified dermatologists or provider at Integrated Dermatology in Washington DC.

“I see this in my practice where I manage negative result or complications from receiving treatments in non-medical settings,” says Dr. Chang. “Board-certified dermatologists are experts in the areas of injectables. Choose your injector carefully.”

Before receiving any BOTOX® treatment, look at your provider’s credentials and experience. Most reputable providers recommend a consultation before treatment, which allows you to ask questions about the provider and discuss your aesthetic goals. You should feel comfortable with your provider and work together to before any treatment takes place.

           BOTOX® enhances your features.

If injected properly, BOTOX® not only treats “wrinkles,” but also enhances your beautiful features. BOTOX® can be used to subtly brighten the eye area with a “brow lift.” A “lip flip” treatment can enhance your natural lips without having to use fillers. A Nefertiti lift can be used along the jawline to create natural facial definition. “Not only does BOTOX® diminish the signs of aging, it can really help balance natural beautiful facial features,” says Dr. Chang.

PRP Hair Restoration, “Vampire Facials”, and PRP Rejuvenation are popular treatments for thinning hair and hair loss, fine lines, wrinkles and scarring, and targeted rejuvenation. But, how does it work, and more importantly, what is it?

           What Is PRP?

PRP stands for platelet rich plasma, which a component of your blood containing growth factors that aid in the healing and regenerative process. When used for hair restoration, it nourishes the hair follicles to strengthen the existing hair and encourage new hair growth. When used for rejuvenation or in conjunction with microneedling, it stimulates your natural collagen production by delivering a concentrated dose of growth factors.

Because platelet rich plasma works by boosting your natural hair growth and collagen production, it needs time to work. Integrated Dermatology’s Victoria Hatfield, PA-C, explains: “It takes time for collagen remodeling after PRP injections, so I tell my patients they can expect to start seeing results within several months after their treatments.”

Patients should also expect to have a series of treatments to get optimal results.

How Long Have People Been Using This Treatment?

Platelet rich plasma treatments were created in the 1970s by hematologists to treat thrombocytopenia, a blood platelet disorder. They were able to isolate the platelets from whole blood and increase the concentration by using a high velocity centrifuge. In the 1980s and 90s, several different medical disciplines, such as maxillofacial surgeons, orthopedic surgeons, and cardiac surgeons, began using this technique to improve wound healing, reduce inflammation, and reduce scarring.

Dermatologists began incorporating these treatments into their practice for acne scarring and fine line reduction. In addition, in 2006, a clinical study showed the efficacy of PRP treatments for alopecia, at specific concentrations. PRP treatment for hair loss has been very promising. “It is exciting to be able to offer an alternative treatment for hair loss,” says Dr. Agnes Chang, Medical Director for Integrated Dermatology in Washington DC. “Currently I prescribe Propecia and recommend Rogaine, but patients are eager to receive treatment that doesn’t require surgery or a pharmacological drug. Having said that, we need more data on the long-term use of PRP.”

Who Is a Good Candidate for PRP?

The short answer is it that depends. For hair restoration, there are many factors that play a role in hair loss and hair thinning. Our providers recommend consultation to review the medical history and perform an evaluation before scheduling treatment. A full work up for causes of hair loss is paramount before receiving PRP treatment.

This is also true for other conditions such as scars and wrinkles. Depending on the type of scar and the current state of skin, previous treatments, and other factors will determine how much improvement is expected from PRP treatment. Many times, multiple treatment modalities are necessary to address the color, texture, and appearance of the skin. This can range from topical treatments to laser treatments in addition to the microneedling or PRP injections.

How to Keep Your Skin Healthy During Even the Harshest DC Winters

The holidays are over and winter is here! The dermatologists at Integrated Dermatology in Washington DC have shared seven secrets to keep your skin healthy in the harsh weather:

Change your skin care routine. During the winter months, your skin tends to be very dry, which can cause “winter itch.” This is especially true if you are prone to eczema. It is important to take lukewarm showers with hydrating cleansers such as Dove Deep Moisturize Nourishing Body Wash and avoid prolonged time in hot water. “You must moisturize your skin immediately following the shower to lock in the moisture with an emollient,” says Integrated Dermatology Medical Director Dr. Agnes Chang. “Otherwise, the low humidity in the air will pull the water from the skin into the air, leaving your skin very dry.” Among Dr. Chang’s favorite moisturizers are Cerave Daily Moisturizing Lotion and SkinMedica Advanced Firming Lotion.

Take a break from harsh or irritating products. During winter, prescriptions and products that you tolerated well in the summer may become irritating. Since the winter air is so dry, you may need to cut down on irritating products, and may need to bump up on skin hydration. Our advice is to decrease the frequency of your exfoliating cleanser or your retinoid cream, and increase the frequency of your moisturizer.

Introduce hyaluronic acid. We recommend adding hyaluronic acid into their regimen. This helps many patients tolerate their Retin-A’s and exfoliating cleansers. Dr. Chang personally uses the SkinMedica HA5 because it has 5 different types of HA5 that are clinically proven to increase your skin’s hydration for up to 8 hours following application. She recommends Neutrogena HydraBoost as an alternative in a pinch.

Start laser hair removal now. Although the warm weather is months away, we recommend starting the process now. More often than not, patients don’t realize they cannot be tan for two weeks leading up to or following treatment. To avoid this, Dr. Chang advises her patients to schedule their complimentary consultation in January and start treatment in February.

Drink plenty of water. The human body is composed of 60% water, which is essential for vital organ functions and an absolute must for vital healthy skin. The rule of thumb is to drink half of your weight in ounces a day. For example, for someone who weighs 120 lbs, you need to drink a minimum of 60 oz. of water per day. “Not only is it important to hydrate your skin from the outside in with moisturizers, but it is also important to nourish your skin from the inside out,” says Dr. Agnes Chang.

Bring travel size products to the gym. Many people resolve to go to the gym more this time of year, but Dr. Lauren McKelroy says this can lead to breakouts if you don’t take preventative measures. She keeps a travel sized SkinMedica AHA BHA cleanser in her gym bag, which helps to prevent and treat break outs. For sensitive or dry skin, she recommends the Vanicream Gentle Facial Cleanser. “The most important thing,” she believes, “is cleansing before you leave the gym.”

Start fresh this year with healthy skin goals! Dr. Agnes Chang suggests consulting with a board-certified dermatologist to discuss your skin health goals for 2020. “As we set our New Year’s goals, it is important to put self-care as a top priority,” she says. “Putting your health first, including your skin health, as a top goal is important for the vibrant life that you would like to lead.” Whether you have a medical concern such as skin cancer or want a good anti-aging strategy, a consultation with a board-certified dermatologist is recommended.

All of us have woken up with a huge zit and have had to suppress the urge to pop it. But, would popping it really be that bad? Washington DC’s leading dermatologists at Integrated Dermatology weigh in on the pimple popping debate:

, PA-C, advises her patients not to pick at or squeeze their pimples. “The chance of scarring or an infection is not worth it,” she warns. In addition to scarring and infection, picking the skin can cause hyperpigmentation, which are red-brown discolorations at the acne sites. “Hyperpigmentation can last several weeks or even several months in darker skin types, and can be challenging to treat,” says Dr. Agnes Chang, Medical Director of Integrated Dermatology in Washington DC.

Dermatologist Dr. Lauren McKelroy says trying to pop something that is not a pimple is a common mistake. “People often try to pop spider angiomas or milia thinking they are pimples,” she says. “Spider angiomas are red dilated blood vessels, which do not pop and often bleed when irritated. Milia are small white bumps with no surrounding redness, which also do not pop. Picking at either of these can lead to scarring.” It is better to seek the advice of a board-certified dermatologist for skin lesions that are symptomatic or don’t seem to heal.

If you must pop, use a Q-tip.

For pustules that have formed a head and are painful, Dr. Agnes Chang says, “Gently cleanse the area. Then squeeze the pustule with a Q-tip just until the contents come out.”

specifies that patients should apply firm steady pressure for 3 seconds. If a pimple does not “pop” within 3 seconds, avoid the compulsion to continue the process. Continued squeezing or picking a skin lesion will only lead to increase in inflammation and ultimately makes the area look worse.

Instead of taking matters into your own hands, make an appointment with a board-certified dermatologist for a small injection. A small amount of cortisone into the pimple can speed the healing process and help it heal faster to avoid the risk of scarring. “These pimple killer shots can be so effective it can resolve a pimple as quickly as 24 hours,” says Dr. Chang.

If you suffer from chronic acne, it is important to make it a priority to treat the acne. Whether it is using an over-the-counter acne cream, watching your nutrition and stress level, or getting started with an effective skin care regimen, it is important to take control of your skin. Work with a skin expert to get a full assessment of your skin and come up with a good treatment plan.
